Insurance - Do you take mine?
In short - Yes! We are a non-profit agency, and we accept all insurance. This includes:
- Private Insurance (in Network for Blue Cross Blue Shield, Healthlink, and Health Alliance)
- Medicare
- Medicaid
- Medicaid MCOs (Meridian, Molina, BCBS, Illini/Aetna, Healthchoice)
- Youthcare and AllKids

Sliding Fee Scale
CRC has a sliding fee scale based on your household income.
When you come to your first appointment, we will ask you to bring a paystub, Social Security Award letter, a W2, or other proof of income so that we can let you know what your copay will be - before your first visit! Your sliding fee scale can be lower than your insurance's listed copay. We honor whichever fee is lower.

What do I need to bring to my first appointment?
Glad you asked! We will ask to see the following:

- Photo ID
- Social Security Card
- Proof of Income
- Insurance Card(s)
- List of prescriptions

If you are referred by an outside agency, please bring a copy of your school IEP/504, DCFS service plan, probation orders, or your DUI evaluation.

How do I make an appointment?
Please call one of our offices. We will ask some questions to get you set up in our system.

First, you will be scheduled an appointment to do your intake paperwork, as well as start your initial assessment.
​
Once your assessment is completed, you will be assigned a counselor.
