Community Support Specialist
Case Manager

Experience/Education:

• Bachelor's in Counseling, Social Work or related field required, or Other Bachelor's Degrees with at least two years supervised experience in the field, or High School equivalent with five years supervised experience in the mental health field or a Certified Recovery Support Specialist.

• Knowledge and training in Mental Health and Substance Use Disorders/Diagnosis

 

Responsibilities

Manage caseload of clients by:

• Conducting assessments

• Assist clients in developing treatment plans

• Document services in Electronic Health Record

• Refer client to necessary agency and community services

• Assist clients with accessing necessary public benefits

• Conduct home visits

• Coach clients and assist them in learning Daily Living Skills

• Coach clients and assist them to learn Recovery/Coping Skills

• Assist clients to accomplish their recovery goals

• Participation in after-hours crisis coverage rotation
